True Confession from the 2012 AAD Meeting

True Confession! I was never going to spend another dollar on a skin tightening device but now my staff and I are training and perfecting our understanding and technique with the Pelleve™. Here is what happened. I heard references about the Pelleve™ at the fall ASDS meeting. I was recently contacted by an old salesman “friend” who was involved with selling me my love, the Candela V-Beam laser. He now works for Ellman, the company behind the Pelleve™. He came and demonstrated the Pelleve™ in my office with me being the patient and Connie and Whitney providing the treatment to my cheeks. It is based on heating the dermis to stimulate new collagen. It felt like the heating pad on high. The actual skin temperature is measured during the procedure to be sure your skin is receiving more than a hot massage so it will stimulate the new collagen production. The results have been studied over a 3 month period showing an increase in collagen and the improvement lasting up to a full year. The Europeans, who have had this treatment for over 7 years, have found the best program is to have one treatment for every decade of your birthday. As usual, the best candidates are in their late 30’s and 40’s but I have seen a few cases of 70 year olds in the published literature. As usual, every treatment works better if there is a major focus on UV radiation protection. (i.e. the sun and every month of the year and cloudy days and through window glass)

In the symposia on Aging Carefully, we heard the new regulations on sunscreen will be in place starting June 2012. Old products will not be pulled from the shelf so read your labels and look for broad spectrum references.
Chemical peels are coming back into popularity particularly the blended peels composed of a lower percent of our popular peeling agents such as trichlorectic acid, phenol, glycolic, and salicylic acids. These can be done on people who can’t stay home for a week to recover. The skin may look and be flaky similar to a sunburn. The catch is the need to do a series of at least 4-6 to see close to the same results as a deeper peel.

Retin A and its variations are still the best topical anti-aging treatment If used with good sun protection. There are several variations and tricks to increase toleration.

In the discussion group entitled, How to Treat the Older Lip (without the duck) and a focus session, Building Beauty: Understanding Facial Proportions, Phi & Use of Volumizing, the fact of consistent proportional comparisons in what constitutes a beautiful face were described and documented. It was fascinating how many of these concepts were repeated in other forms of nature such as flowers and sea shells. Leonardo da Vinci was the master of the concept and it is well identified in his works of art. It most likely is the cause of our discomfort with fish lips and baked potato cheeks.

More to come but I will confess that another fact was brought home to me at this meeting. I can no longer survive going to meetings straight through from 7 a.m. to 6 p.m., surviving on coffee and Power Bars. This meeting is so rich with up to date information, it is hard not to want to attend every lecture. It is fortunate that journal articles and DVDs will follow.

Highlights of the 2012 American Academy of Dermatolgy Meeting

The American Academy of Dermatology 2012 was jam packed with up to date information and nearly 20,000 attendees including many from other countries. It was encouraging to see many younger faces.

Many Highlights:

Elizabeth Blackburn MD PHD, a Nobel prize winner and the recipient of the AAD’s Lila Gruber Memorial Cancer Research Award, gave us an insight to the importance of our chromosome’s telomeres (reminds one of the tips of the breast cancer ribbon logo). The telomeres function as protective caps to the ends of our chromosomes and telomerase is an enzyme that restores and protects the telomeres. This is of major importance because the shortening of the telomeres is the source of our biological aging and the increase risk for major diseases as we age. So what affects all of this? Guess what? Our genetics and environmental and psychological stresses. Bottom line? Since you can’t pick your ancestors, pick your environment (i.e. stay out of the sun, eat healthy, exercise and “don’t worry, be happy” no matter what your life challenges are).

Jean and Alastain Carruthers MD’s received the Eugene J. Van Scott Award for Innovative Therapy of the Skin for their leadership and tenacity to give us the knowledge and ability to use botulism neuro toxin to quell our furrowed brow. They co-presented a humorous and touching account of their trials and tribulations. The lecture was entitled, “You want to inject what?”
